It would seem fairly clear that it was more than just hitting her head.

I expect though that the idea of going down that road was an attempt to snuff out the fuse on what could become an uncontrollable situation, with all the consequences that that entails

It's not that strange for the husband to refuse an autopsy -- what's the point? Obviously she did not die of natural causes. I'm not sure about Pakistan, but in India, immediate cremation and a ceremony adding the ashes to the Ganges.
Do you know what they actually do in an autopsy?
